The Department
The Caulfield Community Health Service (CCHS) is co-located on the Caulfield Hospital site, and provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided in the centre, community, home or via Telehealth. Following assessment, the work will largely involve equipment prescription, home modifications, and referral and liaison with other services to increase or maintain independence and reduce risk in the home. The majority of the clients will be over 65 and referred via My Aged Care under the Commonwealth Home Support Program.
The Role
The Occupational Therapy position will sit within the Adult Health Team and you will:
- Provide comprehensive assessments, recommendations and OT services to clients living at home
- Develop and implement person centred care plans
- Work within a multi-disciplinary team to ensure a holistic approach to client support
- Liaise with relevant agencies to coordinate, facilitate and communicate client treatment and outcomes.This could include local councils and contractors to facilitate installation of home modifications, and equipment suppliers
- Demonstrate an understanding of pathways within the health care system in order to help facilitate appropriate care and advocate for clients with complex care needs (e.g. My Aged Care, Commonwealth Home Support Program, Home Care Packages)
Qualifications and Experience Required
- Degree in Occupational Therapy

About the role
You'll work collaboratively and proactively through telehealth to help customers stay safe and independent at home. You'll triage clinical concerns with care, confidence, and clarity, providing expert clinical input to guide decisions.

Key Responsibilities
Effectively deliver on all requirements of the Clinical Roster including:
- Manage the clinical Inbox and task list
- Triage the customer health and wellbeing notes for identification of, and response to, deterioration
- Completion of Validated Assessment Tools (VATs)
- Assist the other HomeMade teams via booked sessions to deliver clinical guidance and advice
- Complete referrals for Comprehensive Nursing Assessments (CNAs)
- Complete clinical reviews, formulating recommendations to promote health, safety and wellbeing for our customers, and supporting customers and their representatives to understand the recommendations and to make informed decisions to accept or decline recommendations
- Review and respond to clinical escalations, and to incidents and complaints with a clinical component
- Provide guidance and support to customers and their representatives on palliative care and advance care planning
- Provide subject matter expertise and guidance on HomeMade clinical policies and procedures to contracted service providers
- Uphold delivery of services in accordance with the Aged Care Act and Aged Care Quality Standards
- Participate in, contribute to and implement clinical quality improvement and clinical risk management initiatives, as allocated by the Clinical Manager.
- As directed by the Clinical Manager, provide clinical subject matter expertise and support to wider business.
- Maintain practice alignment with the Clinical Governance Framework at all times
- Maintain AHPRA registration and CPD requirements.
- Work with the Clinical Manager to recognise opportunities for service innovation and take a proactive approach to implementation.
- Promote a culture of best practice and shared knowledge by sharing your deep experience in the home care sector and the broader aged care system.
Your skills and experience
- AHPRA Registration as a Registered Nurse
- First Aid certification
- Experience in Home and Community Care, ideally with aged care clients
- Strong clinical decision-making and case management skills
- A warm, human approach and genuine passion for empowering others
- Excellent communication and customer service skills
- Bonus points if you know your way around self-managed or consumer-directed care

Are you passionate about sports and helping young children stay active? Join our team at Amazing Athletes JumpBunch of Southern CT as a Youth Sports Coach for toddlers and preschoolers! We are currently hiring for school year/year round positions with the goal of starting training in early/mid August. Applicants must be available throughout the school year in order to be considered for a position.
In this role, you'll have the opportunity to teach children ages 18 months to 6 years the basic fundamentals of various sports in a fun and non-competitive environment. Our goal is to encourage healthy habits and build confidence in young athletes while ensuring they have a great time!
This position offers flexibility with part-time hours and competitive pay. Classes are typically held in the mornings and early afternoons, allowing for a balanced work-life schedule.
- Must be reliable to work a set schedule, but can be flexible on the days of the week. Coaches work primarily in the morning and early afternoon.
- Experience working with toddlers and preschool-aged children (professional experience is preferred but not mandatory).
- A positive attitude and a love for sports and teaching young children.
- Must be at least 18 years old and possess a clean criminal background check.
- Reliable transportation is required for traveling to different locations within the community. This job requires traveling of upwards of 30 minutes or more. Long distance travel will be reimbursed.
- CPR certification is a plus, though not required.
- Ability to maintain a positive relationship with parents, teachers, and staff at daycare/preschool locations.
