Care Partner - what's involved in the role?
We have Full-time positions available, Monday- Friday in the HammondCare at Home Team Nowra Office, managing our clients during our growth phase.
•The Care Partners assist in providing services tailored to the needs of individual clients, delivered within budget and service expectations, including ongoing client assessment and the development of care plans in consultations with key stakeholders.
•Responsible for care management of existing clients and client-related KPIs
•Ensuring the care plans are up to date for our clients, and they’re receiving an appropriate amount/type of care that corresponds with their need, approval and funding
•Provide professional and competent day-to-day delivery of services to clients in line with service expectations and work to ensure that staff have a respectable understanding of our individual clients’ needs and preferences.
What do you need to apply?
•Certificate IV in Aged Care & a minimum of 2 years relevant industry experience
•Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme
•Experienced in operating effectively in a multi-site environment
•A current driver’s licence and willingness to travel between HammondCare’s clients, sites, and services (salary package can include a packaged motor vehicle on request)
•Strong administrative and computer skills.

About the role:

Role requirements:
With a solid background in aged care, your leadership skills have seen you grow and develop similar sized services with a focus on quality and person-centred care. About the Role
The Disability Access and Inclusion Plan (DAIP) Project Lead will coordinate, monitor and report on the implementation of LCLHN’s Disability Access and Inclusion Plan and Autism Action Plan (AAP), ensuring commitments progress within established timeframes.
Working across the organisation, you will collaborate with leaders, staff, carers, consumers and key external partners to advance disability and autism initiatives. You will support action leads, coordinate the DAIP Working Group, develop project documentation and reporting, and build strategic partnerships that strengthen inclusive practices and improve services for people with disability and/or autism.
This is an opportunity to contribute to meaningful, organisation-wide improvements in disability access and inclusion across LCLHN.
About You
You are an organised and collaborative project professional with strong communication, stakeholder engagement and relationship-building skills. You understand the issues affecting people with disability and can work effectively across diverse stakeholder groups to progress projects, influence outcomes and support meaningful improvements in disability access, autism inclusion and service delivery.
To succeed in the DAIP Project Lead role, you will:
- Demonstrate experience managing projects to successful outcomes, including project documentation and reporting.
- Bring excellent written and verbal communication and influencing skills, including quality report writing and the preparation of policy submissions and funding applications.
- Build rapport and effective working relationships with a broad range of internal and external stakeholders.
- Demonstrate a strong understanding of the current policy context, issues and services impacting people with disability, together with an understanding of equity and disadvantage in the community.
- Effectively manage time, priorities and competing demands to achieve specific objectives and deliver priority projects.
A degree or diploma in social sciences, community development, health or health promotion, social work or a similar discipline and/or experience working with people with disability and organisations is desirable. Experience engaging people with disability and disability organisations and developing initiatives that advance disability access and inclusion is also desirable. People with disability and/or autism are strongly encouraged to apply.

About the role
Join Eyre and Far North Local Health Network as a Clinical Nurse - Community Mental Health, providing high-quality, person-centred mental health care to consumers and their families across the community.

What You'll Bring
- Are you a Registered Nurse with at least three years’ post-registration experience, who holds or is eligible for registration with the Nursing and Midwifery Board of Australia and has, or is undertaking, a recognised mental health qualification?
- Do you have demonstrated competence in mental health nursing, with the ability to provide person-centred, consumer and family-focused care and implement appropriate therapeutic interventions?

The Southcare Outreach Service (SOS) provides multidisciplinary care in the community for the ageing population over 65 years of age residing in the Sutherland Shire. The SOS provides short term acute interventions (up to six weeks) by Nursing, Physiotherapy, Occupational Therapy, Social Work, and our consulting Geriatrician to facilitate enhanced patient care and safe clinical outcomes. SOS staff will provide assessment, intervention and referral onto other required services in order to navigate and integrate the patient’s journey between tertiary and primary care.
The Southcare Outreach Service (SOS) provides a 7 day a week service including public holiday coverage, with varied shift times (a mixture of 8hr and 10hr shifts).

Selection Criteria:

The Role

Medical Oncology Advanced Trainee, Nepean Hospital

The main role of trainees will be to provide care to medical oncology outpatients with exposure to inpatient care at times as well. Advanced trainees will be supervised by consultant medical oncologists and work as part of a multidisciplinary team with other cancer care specialties (Radiation Oncology, Haematology, Palliative Care and Psycho-oncology services). Advanced trainees have access to multidisciplinary team meetings across multiple tumour streams across the network. Trainees will have the opportunity to undertake clinical and translational research projects (e.g. we have one of the largest lung cancer biobanks in NSW) and are supported (and funded) to attend national and international meetings to present their work. All our sites, especially Macquarie University Hospital, have very active clinical trial units. Trainees will get exposure to patients on phase I -III clinical trials.
Our formal teaching program consists of a weekly (1 hour) tutorial/lecture in various oncology subspecialties and a monthly journal club. We have monthly quality improvement meetings and research meetings. First year trainees are encouraged to attend the Basic Sciences of Oncology course. Trainees will also be involved in the teaching of junior doctors and medical students and our hospitals are teaching hospitals of the University of Sydney and Macquarie University.
The Advanced Trainee may rotate to other sites in the Network including Macquarie University, Mater and Orange hospitals.
What you'll be doing
- Under supervision, provide a high standard of clinical care to patients in the area of Medical Oncology to uphold quality service delivery whilst building on skills to become competent in the practice of Medical Oncology in accordance with RACP advanced training requirements.
- Assess, investigate, diagnose, treat and provide ongoing management of in and out patients with oncological diseases and perform procedures and use specialty medical equipment and devices to provide high quality patient care.
- Communicate patient-relevant information to patients and their families/carers and all staff involved in the patient's care, to ensure shared decision-making and timely information transfer. This includes establishing therapeutic relationships, escalating issues when required, and respecting confidentiality and privacy.
Essential requirements
- MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia in an appropriate registration category.
- Demonstrated clinical practice as a registered medical practitioner in the last 2 years in a setting appropriate to the needs of the position.
- Completion of at least four postgraduate years and successful completion of the examination requirements of the Royal Australasian College of Physicians, or equivalent, and be eligible to undertake subspecialty training in Medical Oncology in Australia.
- This position is subject to an appropriate supervision level which can be supported by the health agency. If the incumbent’s registration conditions include supervision requirements, the level of supervision must be able to be supported by the health agency for this role.
- Valid Working with Children Check, cleared National Police Check.
- Aged Care Check for Australian Government funded positions
- To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy, this includes any amendments made from time to time.

Your Opportunity:
This is an exciting opportunity to join a team of paediatricians as our paediatric provisional fellow. We are a friendly, innovative and exciting team who are motivated and interested in rural and complex paediatric medicine. We place a strong emphasis in providing an exceptional standard of service to our under-resourced and vulnerable populations. We have a strong focus on education and service development.
The work includes acute and emergency paediatrics, neonatal resuscitation and special care nursery management and outpatient clinics. We have a 6-bed plus 4-short stay bed Level 4 Paediatric Unit that services the Eurobodalla and Bega Valley regions. We support an experienced Emergency Department with around 4000 paediatric emergency presentations per year. We are also the receiving paediatric hospital for surrounding hospitals in Eurobodalla and Bega Valley which totals another 4000 paediatric emergency presentations per year. We provide a Level 3 maternity service and Level 2 “plus” neonatal service that delivers around 300 babies per annum. We also have a varied outpatient caseload with challenging and interesting paediatric medicine and developmental/behavioural medicine. We provide regular specialist outpatient clinics at South East Regional Hospital and through outreach clinics in Eden, Cooma, Narooma and Eurobodalla.
We have a strong emphasis on education and training and there are teaching opportunities across undergraduate, pre-vocational and vocational training, and GP specialist training levels. We also place strong emphasis on education and support to the surrounding hospitals of our region. We have strong links with the Australian National University Graduate Medical School including placement of rural medical students in the Paediatric ward.
The successful candidate will provide: paediatric services across all of the settings described above; support and supervision of Junior Medical staff and medical students; and leadership in teaching and training for junior medical staff, medical students and nursing staff. The candidate will have the opportunity to be ‘first on call’, and opportunities for research, service development and project leadership under the support and supervision of the paediatric senior staff.
The position is accredited with the RACP for Paediatric Specialist training. The trainee will meet requirements for Rural General Paediatrics with no neonatal component and Community/Developmental Paediatrics as part of a 12 month general Paediatric term.

Selection Criteria
- MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia.
- Registered with the RACP as an advanced trainee or equivalent. Those registered with the RACP must have successfully completed the written and clinical components of the FRACP examination or equivalent and preferably completed a minimum of 6 months as an advanced trainee with the RACP or equivalent at commencement of Fellowship. This position is applicable for peer review via the OTP Pathway.
- Minimum 12 months general paediatric experience. Desirable experience includes experience in paediatric and neonatal acute and critical care, and experience in paediatric developmental / behavioural medicine.
- Demonstrated commitment to quality improvement, patient safety and risk management.
- Excellent communication skills and demonstrated ability to liaise effectively with other health professionals.
- Ongoing commitment to further education and professional development and demonstrated ability to teach, with a willingness to participate in the teaching of medical students, junior medical staff, and other clinical staff.
- Understanding of research methodology or research experience and a willingness to undertake a research proposal.
- Demonstrated contribution to the development of hospital services e.g. participation in paediatric junior staff matters, participation in hospital committees, project leadership.
