About the Role
The purpose of the Child and Family Practitioner MAPSS is to engage and support young people referred to the MAPSS program to improve outcomes across areas of safety, health and wellbeing, social and emotional development, education, and learning. The incumbent works in partnership with the young person and their care team to develop and implement high-quality, individually tailored Care Plans, outlining goals which draw upon the young person's strengths and addressing identified needs.
The objective of the position is to support young people to achieve higher outcomes across ecological domains which correspond with the Child and Youth Wellbeing framework, using the Outcomes Star tool to capture the voice of the young person and their needs, and to assess and measure progress across these domains.
Key Responsibilities
- Develop and maintain functional and supportive relationships with young people and their care teams in order to promote positive outcomes.
- Utilise the Outcomes Star tool to complete initial assessment of strengths and needs, capturing the young person's voice and informing goals.
- Complete safety planning with the young person as necessary.
- Coordinate formal review meetings with relevant stakeholders.
- Support and advocate for the young person's safety and best interests at all times.
- Participate in regular supervision with the Senior Child and Family Practitioner and be open to peer supervision and feedback.
- Undertake regular reporting to referral agency outlining progress, barriers and case planning.
- Ensure the necessary Care Plan and risk assessments are developed and implemented.
- Keep case notes and other records as required by LWB and by law.
Skills & Experience
- Recognised Social Work qualifications with eligibility for membership of the AASW and/or a Bachelor of Psychology or other relevant qualification
- Current First Aid & CPR Certificate (or willingness to obtain)
- Demonstrated experience working with individuals, young people/families and communities from a strength-based, culturally appropriate perspective that are part of the out of home care sector.
- Demonstrated capacity to work autonomously in carrying out agreed Care Plans for multiple young people simultaneously.
- Demonstrated experience in working within a multi-disciplinary team.
- Demonstrated experience in managing competing priorities and to deliver high quality outcomes within agreed timeframes.

- Participates in the Pharmacy and Therapeutics Committee either through membership and/or preparation of materials for formulary decisions and policy-making decisions by evaluating and making written recommendations regarding formulary requests, conducting relevant research, writing up a monograph or drug assessment for presentation at the P&T Committee, and providing necessary information to the all members of the health care team.
- Participates in the planning, implementation and evaluation of department therapeutic cost effectiveness and cost restraint programs. Makes recommendations to the Director, Pharmacy Services and/or the Coordinator, Medication Use Evaluation regarding new or revised policies and standards relative to the designated specialty area.
- In collaboration with members of the program team develops, implements and evaluates goals, objectives and outcomes for the program team, including recommendations in the planning and treatment assessment policies and procedures for the designated specialty area.
- Represents the department on multidisciplinary care committees, and at external events such as conferences, and regional planning groups relative to the designated specialty area.
- Provides pharmaceutical care to patients in designated specialty area; reviews patients' charts and through collaboration with patients and other health care professionals identifies, prevents and solves drug related problems. Identifies viable alternatives, desired outcomes and facilitates therapy change and outcome assessment. Monitors patients' therapy and outcomes, records, maintains and assesses medication profiles; provides education to patients by providing drug information pamphlets and giving lectures to patients' families individually and in groups to ensure they are aware of drug status.

Qualifications
Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
Certificate of completion of year 1 and year 2 of a Hospital Pharmacy Residency Program accredited by the Canadian Pharmacy Residency Board (CPRB) supplemented by two (2) years' recent, relevant hospital pharmacy experience including one (1) year's pharmacy experience in the designated specialty area or an equivalent combination of education, training and experience.

Role Summary
The Alcohol and Other Drugs (AOD) Clinician plays a vital role in providing support and intervention for individuals struggling with substance use issues. Based in Warrnambool, Victoria, this full-time contract position provides culturally appropriate support, education, early intervention, and referral pathways for individuals and families affected by alcohol and other drug use. The program focuses on harm reduction, recovery, and improving health and wellbeing outcomes.
The AOD Clinician works in partnership with clients throughout their individual treatment journey, providing flexible, person-centred, and strengths-based support that respects each client's right to self-determination. The role empowers clients to identify their goals, build on their strengths, enhance their social and emotional wellbeing, and improve health outcomes while maintaining connection to culture, family, and community.
The AOD Clinician will work with adults (aged 16 years and over) to provide assessment, therapeutic counselling, case management and care-coordination. This role includes work with voluntary and sanctioned (forensic) clients. Some clients may engage in individual therapeutic counselling, whilst others may require a combination of counselling and case management/care coordination.

- Experience working with Aboriginal individuals, families and communities affected by Alcohol and Other Drug use.
- Demonstrated ability to deliver culturally safe, person-centred and strengths-based Alcohol and Other Drug services, including assessment, case management, counselling (within scope of practice), care coordination and advocacy.
- Sound knowledge of harm minimisation principles, trauma-informed practice and the social and emotional wellbeing framework.
- Demonstrated ability to work collaboratively within a multidisciplinary team and establish effective relationships with internal programmes, external services and community stakeholders.
- Well-developed written, verbal and interpersonal communication skills, including the ability to prepare reports, maintain accurate client records and communicate effectively with a broad range of stakeholders.
- Demonstrated organisational, time management and administrative skills, with the ability to manage competing priorities and maintain accurate documentation.
- Understanding of Aboriginal cultural protocols, kinship systems and gender considerations, and the ability to apply this knowledge in service delivery.
- Demonstrated ability to exercise sound judgement, maintain confidentiality and respond appropriately to clients with complex needs.

An exciting opportunity to join the Acute Allied Health Division of SALHN. Working across medical and surgical areas, you will use your professional judgement to plan, develop, deliver and evaluate Psychology Services, working collaboratively with patients and the multidisciplinary team.
About the role:As the Psychologist for Acute Allied Health, some of your duties may involve:
- Providing high level, evidence-based Psychological, Cognitive and Behavioural/Emotional assessment, diagnosis and appropriate treatment of mental health, cognitive and psychological issues in patients with complex health issues.
- Providing your expert opinion, consultancy and psycho-education services to staff who are supporting patients with complex psychological, cognitive or mental health issues to maximise self-management strategies.
- Establishing and developing internal and external networks of service providers to deliver the best possible patient resources and pathways and to act as a consultant in clinical psychological services.
- Appropriate Degree or equivalent qualification recognised by the PsyBA and general registration with PsyBA and an endorsed area of practice in Clinical Psychology or Health Psychology.
- Significant relevant clinical experience in the delivery of relevant models and practices.
- Demonstrated experience in the effective development, implementation and evaluation of service improvement activities.
- Ability to apply a high-level of competence in designing and implementing evidence-based psychological interventions and treatments, with a broad range of Clinical Psychology clinical skills.
- Ability to apply a high level of competence and extensive knowledge in psychometric assessment, research and program design.
- Ability to problem solve, time manage and apply specialist skills to complex issues relating to client therapy and the psychology service.
