Position Objective

The Clinical Nurse Consultant - Early Intervention Support Team (EIST) / Safely Reducing Restrictive Practices (SRRP) provides senior clinical leadership and specialist consultancy to support the safe reduction of restrictive practices and clinical violence and aggression at Thomas Embling Hospital. The role works across high and medium secure acute and rehabilitation hospital services to promote recovery-oriented, trauma-informed and evidence-based care, while leading initiatives that improve consumer outcomes, staff safety and service quality.

The Clinical Nurse Consultant - EIST/SRRP ensures professional development for Early Intervention Support Team (EIST) nurses and works in partnership with other Clinical Nurse Consultants at the hospital to safely reduce restrictive practices and reduce the risk of clinical occupational aggression and violence.The role is part of a team of CNCs who work collaboratively with the hospital leadership group and other key internal and external stakeholders to develop workforce capability, strengthen therapeutic engagement, support early intervention and de-escalation strategies, and drive continuous practice improvement. The role provides education, mentoring and expert support to clinical and non-clinical staff and teams managing complex behavioural and risk issues to support a safe and effective practice environment.

The Clinical Nurse Consultant - EIST/SRRP is responsible for leading service improvement and change initiatives, implementing evidence-based strategies to safely reduce restrictive practices and occupational clinical violence and aggression and ensuring consistent, coordinated approaches to care that enhance safety, quality and operational effectiveness across the hospital.

Key Responsibilities

• Lead and coordinate the operational delivery and professional development of the Early Intervention
  Support Team (EIST):
  • coordinate regular EIST meetings with relevant line and clinical security team managers
  • oversee EIST nurses' participation in supporting seclusion review and delivering individual and group programs
  • support EIST nurses to develop and use de-escalation, sensory modulation, and distraction techniques
  • ensure EIST nurses understand and apply procedures for escorting consumers on campus
  • ensure EIST nurses provide timely support to unit-based staff and respond to incidents, should there be multiple responses needed, including where secondary responders have attended
• Provide advanced clinical leadership, consultancy and support to nursing and multidisciplinary teams to deliver high-quality, person-centred, recovery-oriented and trauma-informed care.
• Lead and support initiatives that safely reduce restrictive practices and clinical violence and aggression, ensuring care is responsive to consumer needs and promotes least restrictive practice.
• Coordinate and implement evidence-based early intervention, therapeutic engagement and de-escalation strategies to improve consumer outcomes and staff safety.
• Monitor clinical practice, evaluate outcomes and lead continuous quality improvement activities to ensure compliance with legislative, organisational and best practice standards.
• Collaborate with Hospital Leadership, multidisciplinary teams, lived experience representatives and other stakeholders to improve service delivery and achieve clinical and operational objectives.
• Provide education, coaching, mentoring and professional support to develop staff capability in restrictive practice reduction, preventing and managing clinical violence and aggression, and therapeutic care approaches.

Key Selection Criteria

• Registered Nurse with current AHPRA registration and a postgraduate qualification in Mental Health, Psychiatric or Forensic Nursing, supported by substantial post-registration experience in mental health and/or forensic services.
• Demonstrated advanced clinical expertise in mental health nursing, including the application of contemporary evidence-based practice, professional standards and relevant legislation to support consumers with complex needs.
• Proven commitment to recovery-oriented, trauma-informed and least restrictive practice, with experience leading initiatives that safely reduce restrictive interventions and improve consumer outcomes.
• Demonstrated clinical leadership and consultancy skills, with the ability to influence practice, provide expert advice, mentor staff and lead sustainable clinical improvement.
• Advanced knowledge and experience in the prevention and management of clinical violence and aggression, as a subset of occupational violence and aggression (OVA) in a complex health or related environment.

About the Opportunity

The Throughcare Case Manager delivers pre-release planning and support to people in custody prior to their release from Ravenhall Correctional Centre. The role provides post-release support and works proactively with clients by undertaking a case management and coordination function. The Throughcare Case Manager provides services that are person-centred, trauma-informed, strengths-based and responsive to the complex reintegration needs of people transitioning from custody to the community.

The role develops effective partnerships with internal and external stakeholders and contributes to the design and delivery of programs and services at the Community Reintegration Centre.

As the Throughcare Case Manager you will:

• Maintain a caseload of clients with varying levels of reintegration needs, and provide a responsive level of service to meet these needs, including assertive outreach and support
• Provide effective case management and coordination for clients, including the collaborative development of plans identifying post release needs and support that will promote successful rehabilitation and reintegration outcomes.
• Contribute to and assist with the pre-release planning for those on caseload who are eligible for dedicated post-release programs including Nexus, XIT-180 and other supported pathways
• Coordinate and support clients to implement their Individual Reintegration Plans post-release including assistance with housing, education/employment, drug and alcohol and mental health treatment needs, and family and community connectedness.
• Provide a responsive level of service to people identified as requiring support to secure or maintain accommodation
• In consultation with Team Leader, identify, develop and deliver a range of programs and services at the Community Reintegration Centre that effectivelyaddress the reintegration needs of prisoners and enhance and support their positive engagement withfamilies and communities.
• Contribute to a range of innovative approaches to meeting reintegration needs, such as Employment Expo's or other opportunities to enhance post-release outcomes.
• Engage collaboratively withinternal and external stakeholders tooptimise reintegration outcomes for people accessing Community Reintegration Centre services.
• Provide front of house support at the Community Reintegration Centre
• Maintain accurate and timely attendance, case notes, progress and completion records to support outcome monitoring, reporting and continuous program improvement.
• Participate in team meetings, supervision and training to strengthen practice quality, maintain professional capability and improve participant outcomes.
• Work location will be at Geo Group's Bridge Centre, located at 1/548, Bridge Road, Richmond, VIC 3121.

Your Skills & Experience

To be successful in the role of Throughcare Case Manager you will have:

• Tertiary qualification in Social Work, Psychology, Welfare, Human Services or other relevantfields;(or currently undertaking) or equivalent professional experience.
• Demonstrated knowledge and understanding of research and evidence related to positive resettlement andreintegration outcomes for people released from prison.
• Previousexperience in supporting individualswho have been institutionalised, and who may experience issues with adjustment, behaviour,transition,and reintegration.
• Demonstrated ability to work within a trauma-informed framework, with clients with complex needs such as disability,mental health, substance use and homelessness.
• Ability to coordinate access to and/or deliver programs and services relevant to thereintegration needs of those who have been released from prison.
• Ability to develop and maintaincollaborative stakeholder relationships and networks tofacilitate positive outcomes for clients.
• Well-developed interpersonal and communication skills and attitudinal characteristicsconducive to building positive, non-judgemental professional relationships with people with ahistory of incarceration.
• Strong problem-solving and critical-thinking skills, supported by a practical, solutions-focused approach to reducing barriers, responding to emerging needs and improving participant outcomes
• Previousexperience working in a community case-managementrole
• Demonstrated skill in motivational interviewing or other engagement strategies

You will lead embedded unit-based staff, strengthen clinical governance and service coverage, and provide trusted advice on complex behavioural and psychological risk. Working alongside Principal Clinicians and the Principal Clinician, Service Improvement and Outreach Support, you will help deliver coordinated in-reach services and maintain continuity at GEO's Community Reintegration Centre when required.
