As a Registered Nurse in Community Care Coordination, you’ll play a key role in ensuring individuals receive the right care at the right time. Working within a collaborative multidisciplinary framework, you’ll lead comprehensive assessments, coordinate tailored care plans, and connect consumers with the services and supports they need to thrive in the community. ISLHD operate four hospital sites in the Illawarra: Coledale District Hospital, Bulli District Hospital, Wollongong Hospital and Shellharbour Hospital.

5 South Cardiac Ward and Coronary Care Unit (CCU) provides specialised care for patients requiring cardiac monitoring, intervention, treatment, and rehabilitation. Caring for a diverse patient population across acute cardiology and coronary care services, our team is committed to delivering safe, evidence-based, and person-centred care in a supportive and collaborative environment.
As a Registered Nurse, you will be an integral member of a highly skilled multidisciplinary team, providing comprehensive nursing care to patients with acute cardiac conditions, including coronary syndromes, arrhythmias, heart failure, and post-procedural cardiac care. You will have the opportunity to develop specialist cardiac nursing skills while working in a progressive clinical environment that promotes learning, innovation, and professional growth.
What You'll Be Doing
- Delivering high-quality nursing care to patients with acute and complex cardiac conditions
- Undertaking comprehensive patient assessments and developing individualised care plans
- Monitoring and responding to changes in patient condition using advanced clinical assessment skills
- Caring for patients requiring cardiac monitoring and telemetry
- Supporting patients through acute illness, recovery, and discharge planning
- Collaborating with cardiology, medical, nursing, and allied health teams to achieve optimal patient outcomes
- Contributing to a culture of clinical excellence, patient safety, and continuous improvement
- Supporting and mentoring students, new graduates, and junior staff

A little about us.
Shoalhaven Memorial Hospital is the main referral hospital for the Shoalhaven region, providing emergency care, medical, surgical, mental health and orthopaedic services. The hospital has an Intensive Care Unit focusing on ventilated, high dependency and coronary patients, Children’s Ward and Maternity Unit, as well as a purpose-built regional Cancer Care Centre.
The hospital is currently undergoing a significant $440M redevelopment that will redefine how healthcare is delivered within the Region. Once complete in 2026, the new seven-storey Acute Services Building and refurbishments on the current hospital site will bring a focus on career development. allowing you to grow your career, acquire new skills and advance professionally as the hospital continues to develop. Within this redevelopment, a purpose-designed Mental Health Inpatient Unit will open for the first time at Shoalhaven Memorial Hospital, providing local consumers with access to comprehensive inpatient mental health care closer to home. Recent examples of our innovation include the ‘Yarning space’ and improving the inpatient discharge process to transition to in community supports. About You
To be successful in this role, you will have:
- Demonstrated experience and knowledge in the National Immunisation Program, vaccine administration, vaccine preventable diseases, cold chain management and adverse events following immunisation.
- Current immunisation accreditation/competency in accordance with relevant Australian requirements.
- Demonstrated experience in administering vaccinations and strong clinical assessments.
- Demonstrated ability to work within a small team of professionals and to assist colleagues within the Public Health Unit to ensure collaboration.
- The ability to work independently as well as collaboratively within a team.

We are seeking an experienced and motivated Registered Nurse to join our team as a Facility Flow Coordinator, playing a key role in ensuring patients receive the right care, in the right place, at the right time.
Working across nursing, medical, allied health and operational teams, you will coordinate patient flow, support bed management, facilitate admissions and transfers, and assist with timely discharge planning.
No two days will be the same. Using your clinical judgement, leadership and problem-solving skills, you will respond to changing demand, address barriers to patient flow and support safe, efficient patient care.

Your Key Responsibilities
As Facility Flow Coordinator, you will play a key role in leading and coordinating patient flow across Belmont Hospital, ensuring patients have timely access to beds, services and appropriate care.
You will work closely with nursing, medical, allied health and operational teams to monitor capacity, coordinate admissions and transfers, support timely discharge planning and address barriers to patient movement.
Your role will include:
- Responding to changing patient demand and capacity pressures.
- Providing clinical leadership during periods of increased activity.
- Escalating issues and working with key stakeholders to identify practical solutions.
- Using patient flow information to support operational decision-making and communication.
- Contributing to quality improvement initiatives that enhance patient flow, access and patient outcomes.
You will also build strong relationships across the facility, supporting effective bed management, multidisciplinary collaboration and hospital-wide patient flow coordination.

This brand-new, purpose-built facility features 37 inpatient beds, including:

- 5 high-acuity beds
- 4 beds for younger people
- 8 older persons mental health beds
The Tamworth Mental Health Unit delivers contemporary, recovery-oriented care within a modern therapeutic environment designed to support consumers, families and staff.
Care is provided by a highly skilled multidisciplinary team, incorporating medical, nursing, psychology, occupational therapy, social work and allied health professionals. Consumers are encouraged to participate in a structured therapeutic program focused on engagement, social connection, education, skill development and improving both mental and physical wellbeing. As an Undergraduate AIN, you’ll have the opportunity to gain valuable hands-on experience within a specialist mental health setting while building your clinical skills and confidence alongside experienced clinicians. From day one, you’ll be supported through a comprehensive orientation program, regular in-service education and access to ongoing professional development, supervision and mentoring.
What You Will Be Doing
As an Assistant in Nursing, you will support Registered Nurses and Enrolled Nurses to deliver safe, person-centred and recovery-focused care to consumers within the inpatient mental health setting.
Your responsibilities will include:
- Assisting consumers with activities of daily living, including personal care, nutrition, mobility and comfort.
- Undertaking delegated consumer observations and reporting changes in behaviour, mental state or physical health.
- Supporting consumers to participate in therapeutic, recreational and group activities.
- Assisting with routine clinical tasks within your scope of practice.
- Maintaining a safe and supportive ward environment.
- Accurately documenting delegated care and observations.
- Communicating effectively with nursing staff and the multidisciplinary team to support continuity of care.
This role works within a clearly defined scope of practice under the delegation and supervision of a Registered Nurse and reports to the Nurse Unit Manager through the Registered Nurse in Charge.
This is an excellent opportunity to gain practical experience in mental health nursing while working in a supportive, learning-focused environment.
