About the role

As an experienced Level 3 Social Worker, you will provide high-quality assessment, treatment and care coordination service to adults experiencing a range of mental health challenges. You'll deliver high-quality, person-centred care, working closely with individuals on their recovery journey to help them achieve their goals and improve their wellbeing. You will play a key role within a collaborative multidisciplinary team, contributing to a psychosocial perspective to care for mental health consumers.
Your focus will include addressing psychosocial stressors, promoting family and cultural inclusion in care planning, and providing specialist input on matters related to family and domestic violence, as well as child protection. You will be supported in this position by the Community Manager, Clinical Coordinator and Head of Discipline. The successful applicant will become part of a wider Hunter New England local Health District Mental Health Service network and will have access to support and education from key district support positions.

The Aged Care Single Assessment System (SAS) has been set up by the Health Department to simplify and improve access to aged care services. SAS includes assessments for all aged care services, including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments.

About You
To be successful in this role, you will have:

- Liaising with clinical staff/ pharmacy and informing them of residents' planned discharge dates for transport arrangements
- Case management may include supporting clients with State Administrative Tribunal (SAT) and guardianship processes.
- Planning and undertaking social work interventions with clients, in conjunction with their families and the multidisciplinary team
- Providing care coordination, counselling, advocacy, mediation, referrals and support for clients when transferring them to either their home environment or a Residential Care Facility
- Providing consultation and support to Nursing and Allied Health professionals regarding psychosocial aspects of care for clients
- Ensure compliance is maintained in line with Amana Livings policies and procedures, and accreditation standards are sustained.
- provide case management and discharge planning services to ensure that the client can either: remain with the provider as a permanent residential client or, be discharged home (community) with appropriate supports, or be discharged to another provider.
The successful candidate will demonstrate experience working in a fast-paced role in an organised and adaptable manner. Strong interpersonal skills are essential as this role will see you interacting with residents and their families and engaging the multidisciplinary team to provide an autonomous high level of care to meet the needs of our residents and their families.
- Bachelor of Social Work
- Knowledge of the Aged Care Sector
- Strong time management skills and ability to prioritise tasks efficiently
- Excellent problem-solving and negotiations skills
- High attention to detail
- Exceptional verbal, written and interpersonal communication skills
- Evidence of Current Flu vaccinations per Amana Living policies
- Ability to obtain a National Police Clearance (within 6-months validity)
- Successful completion of pre-employment medical and reference checks
