In this role, you'll work directly with prospective foster carers, guiding them through recruitment, training, and assessment, and supporting them to become approved carers providing safe, stable, and trauma informed care.

In this role, you'll:
- Support the development and delivery of marketing and attraction activities to engage prospective foster carers
- Promote foster care through community outreach including events, forums, schools, and local networks
- Manage enquiry pathways, including pre-screening and compliance checks for applicants
- Facilitate carer preparation training to equip applicants for their caring roles
- Conduct comprehensive foster carer assessments using competency-based tools
- Present assessments at authorisation panels
- Maintain accurate and up-to-date information across carer management systems
- Build strong relationships with stakeholders, service networks, and community organisations
- Ensure all work reflects trauma-informed practice and the Sanctuary Model
- Identify opportunities for continuous improvement in recruitment and assessment practices
- Travel regularly and work flexible hours, including evenings and weekends when required
Based in Rockhampton and/or Gladstone, we are currently recruiting 1 Foster Care Recruitment Practitioner to support our work across the Central Queensland region. These roles involve regular regional travel and sit within the Queensland Foster Care Recruitment team, reporting to the Foster Care Recruitment Coordinator.
This is a full-time position, with flexibility required including occasional evenings and weekends.
Key Selection Criteria
To be successful in this role, you will have:
- A relevant tertiary qualification in social work, psychology, community services, or a related field
- Demonstrated experience in the Out-of-Home Care sector
- Experience in carer recruitment, assessment, training, or support
- Demonstrated ability to deliver training or facilitate group work
- Strong relationship-building and stakeholder engagement skills
- High-level organisational and time management skills

The Navigator provides expert, autonomous care navigation and coordination for parents experiencing moderate to severe perinatal and infant mental health concerns, facilitating timely access to appropriate services across the South Australian PIMH care continuum.
Key duties of the role- Provide expert triage, care navigation and referral support to connect clients with appropriate perinatal and infant mental health and community services.
- Actively participate in practice level multidisciplinary team meetings.eg. Community of Practice
- Hold a contemporary professional practice portfolio containing evidence of qualifications, learning and practice experience that underpin and demonstrable application of knowledge and skills commensurate with the level and type of practice expected of the role.
- Contribute towards the development of the project, attending meetings and undertaking presentations.
- Provide leadership and direction, acts a role model, mentor, consultant, and resource person relating to PIMH.
- Registered Nurse and/or Midwife with current registration with the Nursing and Midwifery Board of Australia, or an eligible Social Worker or Psychologist.
- Minimum 3 years' post-registration experience in nursing, midwifery, social work or psychology.
- Demonstrated experience and competence in perinatal and infant mental health practice.
- Demonstrated ability to undertake biopsychosocial assessment, risk assessment, care planning and intervention.
- Strong communication, relationship-building and multidisciplinary collaboration skills, including knowledge of Aboriginal communities and culturally responsive practice.

About the Role
Join the Casey Adult Crisis Intervention Service (ACIS) as a Grade 4 Mental Health Clinician and play a key role in delivering timely, recovery-oriented care to consumers experiencing an acute mental health crisis. This is a full time, ongoing position, based in Cranbourne.
Working across the community, clinic and via telehealth, you will undertake comprehensive mental health, psychosocial and risk assessments, provide crisis intervention, and develop collaborative treatment and safety plans. You will work closely with consumers, families, carers and a broad multidisciplinary network to support people to remain safely in the community wherever possible.
This is a senior clinical role where you will apply your expertise in mental health legislation, risk assessment and crisis management while partnering with inpatient units, emergency departments and community agencies to ensure seamless care and positive consumer outcomes. The position operates across a seven-day rotating roster including AM and PM shifts.
About the Team
The Casey Adult Crisis Intervention Service (ACIS) forms part of the Casey Area Mental Health Service, supporting one of Melbourne's fastest-growing communities. We provide intensive, short-term assessment and treatment for adults experiencing an acute mental health crisis, delivering care in consumers' homes, within our clinic and via telehealth.
Our team is committed to recovery-oriented, person-centred care and works collaboratively with consumers, carers, families and a wide network of health and community partners. Supported by experienced and approachable leaders, you'll join a highly skilled multidisciplinary team that values clinical excellence, innovation and continuous learning.
About You
You are an experienced Mental Health Social Worker or Registered Nurse with a passion for delivering high-quality, recovery-focused care in fast-paced environments. Confident working with complexity, you bring advanced assessment, crisis intervention and risk management skills, along with a sound understanding of the Mental Health and Wellbeing Act and contemporary mental health practice.
You are a collaborative clinician who builds strong relationships with consumers, families, carers and multidisciplinary teams, and you thrive in a role where no two days are the same. Your excellent communication skills, sound clinical judgement and commitment to person-centred care enable you to confidently navigate complex presentations while supporting safe and effective outcomes.
To succeed in this role, you will also bring:

What You Will Be Doing
As an Assistant in Nursing, you will support Registered Nurses and Enrolled Nurses to deliver safe, person-centred and recovery-focused care to consumers within the inpatient mental health setting.

About the role
The Senior Clinical Coordinator provides specialist clinical expertise in the coordination and planning of multidisciplinary assessments for children with complex neurodevelopmental concerns. The role leads the development, coordination, implementation, delivery and evaluation of high-quality, evidence-based assessment and management services. The position provides advanced clinical assessment and support to children, adolescents and their families experiencing complex developmental differences.
Key duties of the role
- Manage the delivery of high quality and evidence-based services to children presenting with complex developmental differences in three or more areas of their development
- Have knowledge of child development and of the range of services available for children and their families within South Australia that promote child development
- Contribute to the effective and professional functioning on the Child Development Unit

Skills and Experience

About the Opportunity
An exciting opportunity is available for a Registered Nurse (Mental Health) to join the Newcastle or Lake Macquarie Community Older Persons Mental Health Service (OPMHS).The Community OPMHS is a specialist service providing assessment, treatment and ongoing care to older people experiencing mental illness, cognitive concerns and complex psychosocial needs.
We have two opportunities available, one at Lake Macquarie Community OPMHS is based at Charlestown Health Campus and provides specialist community mental health care across the Lake Macquarie area. Newcastle Community OPMHS is based at Watt Street, Newcastle, on the former James Fletcher Hospital site and provides specialist community mental health care across Newcastle and surrounding areas.
Both services are multidisciplinary teams comprising nursing, medical, psychology, social work and occupational therapy clinicians. You’ll work collaboratively with consumers, families and carers, GPs, residential aged care facilities and other health and community services. Our teams are committed to person-centred, recovery-oriented care, supporting older people to remain well and independent within their own homes and communities wherever possible. This means your work will extend beyond the office, with regular opportunities to undertake home visits, residential aged care facility visits and community-based assessments, while remaining connected to a supportive multidisciplinary team.
What You Will Be Doing
As a Registered Nurse within the Community Older Persons Mental Health Service, you’ll provide specialist nursing care across assessment, treatment, care coordination and ongoing clinical management.
You will:
- Undertake comprehensive mental health assessments, including mental state examination, psychosocial assessment and risk assessment.
- Provide care coordination for consumers with complex mental health and psychosocial needs.
- Develop, implement and review collaborative care plans with consumers, families and carers.
- Monitor mental state, treatment response, medication adherence, physical health and changes in risk.
- Provide nursing interventions, psychoeducation, medication support and other evidence-based interventions within your scope of practice.
- Undertake home visits, residential aged care facility visits and assessments across community settings.
- Work closely with psychiatrists, medical officers, GPs, families, carers, aged care providers and other health and community services.
- Participate in multidisciplinary clinical reviews, case discussions and treatment planning.
- Support consumers experiencing deterioration in their mental health and contribute to timely assessment and escalation.
- Contribute to clinical governance, quality improvement and service development activities.
- Build therapeutic relationships that support recovery, engagement and positive outcomes for older people.
The role offers a high degree of professional autonomy, balanced with ready access to experienced nursing, medical and allied health clinicians when you need advice or support.
About You
We’re looking for a compassionate, reliable and clinically curious Registered Nurse who enjoys working with older people and is interested in supporting consumers with complex mental health, physical health, cognitive and psychosocial needs.
You will bring:
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
- A current Australian driver's licence, as the role involves community visits.
- Sound foundational clinical skills and professional judgement.
- Strong communication and interpersonal skills.
- The ability to build therapeutic relationships with older consumers, families and carers.
- The ability to recognise changes in mental state and risk and respond appropriately.
- Strong organisational skills and the ability to prioritise a community caseload.

Previous mental health nursing experience is highly regarded, however, specific Older Persons Mental Health experience is not essential.
We welcome Registered Nurses with strong transferable clinical skills and a genuine interest in developing their practice in community and Older Persons Mental Health.
With support from the team, you’ll have the opportunity to develop specialist knowledge and skills in mental health assessment, risk assessment and management, care coordination, cognitive assessment, mental health legislation, aged care systems and local community services.
How You Will Be Supported
You’ll receive comprehensive orientation and ongoing support from the Team Manager, Clinical Coordinator, senior clinicians, medical staff and experienced multidisciplinary team members.
Your orientation will be tailored to your previous experience and will include the OPMHS model of care, local service processes, clinical documentation, community practice, risk assessment and management, relevant legislation, referral pathways and local community services.
Ongoing support includes:
- Regular clinical supervision.
- Multidisciplinary clinical review and case discussion.
- Joint community visits and assessments during orientation where appropriate.
- Access to experienced mental health clinicians for consultation.
- Education and professional development opportunities across the broader Hunter New England Mental Health network.

The Outpatient Rehabilitation Service
Outpatient Rehabilitation (OPR) is a multidisciplinary allied health outpatient service for patients residing in the THS-North returning home to the community following an inpatient hospital admission. The service is based at the Launceston General Hospital. The Outpatient Rehabilitation team includes Allied Health Assistants, Dietetics, Clinical Psychology, Occupational Therapy, Physiotherapy, Social Work and Speech Pathology. OPR aims to provide goal-directed, time limited rehabilitation for patients with physical, psychological, emotional or cognitive difficulties and deficits arising from their recent medical event. The caseload includes patients with stroke, traumatic brain injury, amputation, deconditioning and other conditions requiring rehabilitation after a hospital admission.
The Role
The Allied Health Team Leader- Outpatient Rehabilitation provides clinical leadership and oversees best practice multidisciplinary rehabilitation. This is an opportunity to step into a key clinical leadership role and shape the future of the Outpatient Rehabilitation service at Launceston General Hospital. The role combines hands-on expert clinical practice with leadership, service development, education, and innovation across the service. This position plays a pivotal role in enhancing patient outcomes, supporting staff development, and driving excellence in care delivery.

Fixed term, part time, day worker position, working 30.4 hours per fortnight, commencing as soon as possible until 30 January 2028.

About the Role
Join Monash Health's Crisis Assessment and Treatment Team (CATT) as a Grade 4 Senior Clinician. This opportunity is a full time ongoing position and is open to Social Workers who are passionate about delivering high-quality, recovery-oriented mental health care.
Working within a multidisciplinary team, you'll provide comprehensive mental health assessment, crisis intervention and short-term treatment for consumers experiencing acute mental health concerns. The role combines community outreach with onsite assessments and works closely with consumers, families, carers, emergency departments and community partners to support people to remain safely in their community.
As a senior clinician, you'll also provide clinical leadership, secondary consultation, supervision and mentoring, while contributing to service improvement, education and evidence-based practice.

An exciting opportunity is available for a Grade 3 Social Worker to join Monash Health’s Secure Extended Care Unit (SECU) at Dandenong Hospital in a full-time, ongoing position. You’ll manage a varied caseload, working closely with consumers, families and carers to support recovery and address the broader social factors that can impact successful community transition. In this role, you will:

The Monash Health Gender Clinic is Australia's longest-established gender service, providing statewide care for transgender, gender diverse and non-binary (TGDNB) people. Our multidisciplinary team delivers gender-affirming care, peer support, education and advocacy, while working to improve access and outcomes for TGDNB communities. This role works within a collaborative team of peer workers, psychology, psychiatry, social work, speech pathology, endocrinology and administration professionals.
About the Role
Join our team as a Consumer Peer Worker Level 3 on a part-time (0.6 EFT, 3 days a week), ongoing basis at the Monash Health Gender Clinic, located within the Victorian Pride Centre in St Kilda.
Drawing on your lived experience as a trans, gender diverse or non-binary person, you will provide intentional peer support to consumers accessing the Gender Clinic and contribute to a supportive, recovery-oriented service.
In this role, you will:
- Provide one-to-one peer support using lived experience and Intentional Peer Support principles.
- Support consumers to navigate health, community and LGBTQIA+ services.
- Promote self-advocacy, wellbeing and informed decision-making.
- Facilitate peer-led groups and contribute lived experience perspectives across the service.
- Support and mentor other peer workers and contribute to a positive team culture.

The Grade 3 Social Worker is responsible for the provision of Mental Health Social Work services at the Acute Inpatient Service. The Social Work Grade 3 Clinician is expected to work independently whilst at the same time contributing strongly to the multidisciplinary mental healthcare team, and to initiate and participate in teaching, training, research and quality activities.
- Work under the Mental Health and Wellbeing Act 2022 to skilfully engage consumers, carers, families and supporters

- Work within MARAM and Information Sharing framework
- Liaise with other St Vincent’s Mental Health services and external agencies to ensure effective and efficient discharge is managed
- Liaise with community groups and relevant agencies to develop and review community networks which enhance continuity of care
- Supervise and provide training to social students and other non social work staff according to service policy, University Schools of Social Work and AASW requirements

St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. The adult mental health service components are: The Footbridge Community Care Unit (CCU), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincent’s Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention), Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. Your Role as Mental Health Clinician - Mental Health Multidisciplinary Team:
As a Mental Health Clinician, you will provide assessment, brief intervention, case management and coordinated care to adults with complex mental health needs. Our main site is located at Cranbourne Integrated Care Centre, with additional services delivered across the local catchment. Our interdisciplinary team works together to provide accessible, family-centred care that considers the whole child and the important people and environments around them.

You are a Social Worker or Occupational Therapist who is passionate about improving outcomes for children and families. An exciting opportunity exists for an experienced Grade 3 Social Worker to join Monash Health’s Renal Supportive Care team.
Working within a specialist multidisciplinary service, you will provide advanced psychosocial support to patients living with advanced chronic kidney disease (CKD) and their families and carers. You will support people to navigate complex healthcare decisions, plan for the future and maintain quality of life throughout their care journey.
Based at Monash Medical Centre, Clayton, this ongoing part-time (0.4 EFT) role offers the opportunity to work alongside experienced clinicians across nephrology, supportive care, palliative care and complex chronic disease management.
About the Role
As the Renal Supportive Care Social Worker, you will provide specialist social work assessment, intervention and support to patients and families experiencing complex health and psychosocial needs. You will:

- Support informed decision-making and advance care planning
- Connect patients and families with appropriate health, community and support services

About the Service

With a strong focus on person-centred care, innovation and continuous improvement, the team supports patients and families through complex treatment decisions and changing healthcare needs.
About You
You are an experienced Social Worker with advanced clinical skills and a compassionate, person-centred approach. You are comfortable working autonomously, enjoy multidisciplinary collaboration and can confidently support patients and families through complex healthcare decisions and significant periods of change.
You will bring:
- A Bachelor of Social Work, Master of Social Work (Qualifying), or equivalent qualification recognised by the AASW
- Eligibility for membership of the AASW
- Advanced psychosocial assessment, counselling and intervention skills
- Strong communication, advocacy and care coordination skills

About the opportunity
Summary of the role: The role of the Forensic Clinical Specialist (FCS) is to build capacity and support best practice within Mercy Mental Health & Wellbeing Services Staff, provide forensic mental health consultancy experience. This position will provide a resource to staff to support and enhance the service response for consumers presenting with multiple complex presentations including serious mental illness, substance abuse issues, forensic history and high risk behaviors. The FCS position will provide timely advice, specialist assessment, and secondary consultation, develop and implement training, and introduce high level evidence based risk assessment and risk management tools and processes. It will be a key contact point for justice / forensic referrers, Mental Health Community Support Services (MHCSS) and MMH and will support a collaborative approach to sharing and transfer of care for this consumer groups.
ABOUT YOU:
- Registered with APHRA as a Psychologist with eligibility for endorsement as a Clinical psychologist or
- Registration with APHRA as an Occupational Therapist with a graduate or post graduate qualification in Occupational Therapy or
- Registration with APHRA as a Registered Nurse undergraduate psychiatric nursing program or a Post Graduate Psychiatric / mental health nursing qualification
- Approved Degree in Social Work with eligibility for membership of the Australian Association of Social Workers.
- At least 5 years post graduate clinical experience in relevant community mental health settings
- Working knowledge of the Mental Health Act 2014, Crimes (Mental Impairment and Unfitness to be Tried Act 1997, Sentencing Act 1991), and other relevant legislation

Your opportunity
Working within our Community & Mission team and reporting to the Program Lead/Senior Family Dispute Resolution Practitioner, you'll manage family dispute resolution matters from initial assessment through to completion.
You'll undertake comprehensive assessments and preparation with clients, identifying issues such as domestic and family violence, safety concerns and power imbalances before determining whether mediation is appropriate. You'll facilitate equitable participation between parties, help clients work through impasses and support them to develop parenting plans and property agreements within the relevant legal framework.

· Deliver high-quality family dispute resolution and property settlement mediation services.
· Manage client matters from assessment and preparation through to completion, including appropriate case planning and documentation.

· Support separating families to develop child-focused parenting plans and appropriate property agreements.

· Maintain accurate client records and ensure practice meets organisational, legislative, professional and funding requirements.
What you'll bring
You're an experienced and compassionate practitioner who can remain calm, impartial and focused when working with people navigating highly emotional and complex family circumstances.
You bring strong mediation and conflict resolution skills, excellent judgement and the ability to recognise safety concerns and power imbalances. You understand the importance of giving both parties an equitable opportunity to participate while maintaining a strong focus on the wellbeing and needs of children.
You'll also bring excellent active listening and communication skills, emotional intelligence, reflective practice and the ability to work sensitively and confidentially with vulnerable clients.
What's required:
· Tertiary qualification in Law, Psychology, Social Work or equivalent.

About the Role
Join our Child & Family Program as a Grade 2 Social Worker and make a meaningful difference for children and families experiencing complex health and psychosocial challenges. Working within a multidisciplinary team, you’ll provide specialist social work assessment, intervention, counselling and advocacy to children and families across the City of Greater Dandenong, City of Casey and Shire of Cardinia.

- Help clients understand their financial position by developing money plans, reviewing income and expenditure and identifying opportunities to improve money management.
- Develop and review client-led case plans, supporting people to set practical goals and build financial knowledge, skills and confidence.
- Provide strengths-based financial coaching that encourages positive behaviour change, problem-solving and long-term financial resilience.
- Advocate for clients and provide warm referrals to services, resources and financial supports that align with their needs.
- Work collaboratively with Financial Counselling, Community Pantries, Hubs and other services to provide holistic support.

What you’ll bring
You’re someone who can talk about money without judgement and understands that financial hardship is rarely just about numbers.
You’ll bring genuine compassion for people experiencing financial stress, together with the practical case management skills to help clients identify their own goals and build the confidence and capability to achieve them.
You’re calm under pressure and comfortable supporting people who may be distressed or experiencing complex circumstances. You can connect effectively with people from diverse backgrounds, including culturally and linguistically diverse communities and Aboriginal and Torres Strait Islander communities.
You’ll also be comfortable working out in the community and collaborating with churches, local services and other organisations to make support easier for people to access. Financial Capability Caseworker.
What’s required:
- Degree in Social Work, Community Services or a related discipline, or diploma/other relevant qualification with substantial relevant experience.
- At least 2 years’ experience working with diverse or disadvantaged communities, including people experiencing financial hardship.
- Financial Literacy Education certification, or willingness to obtain.
- Strong case management skills, including goal setting, monitoring progress and adapting support as client needs change.
- Experience using client databases or CRM systems.
- Strong communication, collaboration and organisational skills.
- Valid Working with Children Check for paid employment.
