About the role As a member of the Integrated Family Services team, the Integrated Family Services Case Manager will work in partnership with families, children and young people toward developing a care plan to achieve mutual goals as well as reduce any risks posed to children and young pe ople's safety and wellbeing.

The focus of this position is the West Family Services Alliance. The position s are based in Braybro ok and Collingwood however the work involves outreach in people's homes . It may involve seeing children and families in other settings such as schools or in the community. Your duties include

• Address protective concerns and enhance family functioning whilst ensuring the safety and wellbeing of the child/children, young people, and family members effective care planning, case practice and ongoing assessment and liaison
• Manage risk according to the "Best Interests" framework
• Liaise and, where necessary, consult with Community Based Child Protection Practitioner (CBCPP) and The Orange Door
• Work with families towards desired change, through direct, goal focussed case practice using a child focussed, family centred approach.

Our ideal candidate:

• Holds the relevant tertiary qualifications related to Social Work, Psychology, or a similar field
• Demonstrated experience in working effectively with people who may have a range of complex needs.
• A well-developed capacity for family assessment, intervention skills as well as the development of care plans in conjunction with the family.
• Demonstrated understanding of childhood development and how trauma, abuse or neglect can impact child development
• Strong belief and personal alignment in cohealth Values and Mission

The Senior Forensic Mental Health Clinician provides specialist and comprehensive mental health assessment, treatment, and support to individuals completing a Mental Health Treatment and Rehabilitation Condition as part of a Community Corrections Order (CCO) or Parole Order.

Leveraging knowledge and experience, this role provides specialist clinical guidance and practice support to a multidisciplinary team.

• Conduct comprehensive, evidence-based biopsychosocial mental health and structured forensic risk assessments in clinical, community, and workplace settings
• Develop, implement and review evidence-based, trauma-informed treatment plans
• Manage a complex and high-risk forensic caseload, ensuring appropriate treatment, risk management, escalation and documentation
• Promote reflective and evidence-based practice, continuous clinical improvement and high-quality forensic mental health care

About the role The purpose of this role is to provide primary care to young people at the Parkville Youth Justice Precinct. This includes comprehensive biopsychosocial assessments and establishing a therapeutic alliance that is respectful of a person's choices, experiences, and circumstances in developing a comprehensive treatment plan. The GP will work collaboratively with the broader multidisciplinary team to deliver client centred, evidenced based, holistic care. Successful candidates will be required to participate in a rotating on call roster.
