The Aged Care Single Assessment System (SAS) has been set up by the Health Department to simplify and improve access to aged care services. SAS includes assessments for all aged care services, including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments.
Working as part of the SAS team, and reporting to the SAS Clinical Team Leader, you will be responsible for:
- Undertaking client assessments, reviews and development of support plans to assist clients in achieving their goals and living independently in their own homes and community across all Northwestern Melbourne LGAs.
- Participating in clinical triage and case conferencing
- Identifying, monitoring and managing risks and leading appropriate escalation.

About Puckapunyal Health Centre
Puckapunyal Military Area is located approximately 13km west of Seymour. This was first used as a mobilisation and training area during WWI and by 1939, and was formally established in the early 1940's. ABOUT THE ROLEAs an experienced Sonographer, you will join being part of our friendly, professional, patient focused team at Ballarat's 2 practices which includes the St John of God private hospital (the largest private hospital in regional Victoria) and our private practice located across the lake. As a progressive and comprehensive service across all medical imaging modalities including Cardiac CT, PET, Nuclear Medicine, MRI, Ultrasound and Mammography, you will be offered with the opportunity to enjoy the variety of cases and ultrasound scanning to ensure that your Sonography career provides the depth that you strive for as you are able to work on state of the art equipment in a fully digital practice, supported by highly specialised and long standing team of Radiologists in the practices daily.

THE ROLEWe are seeking motivated veterinary graduates striving for excellence in equine practice to work in all areas of our hospital. REQUIREMENTS- Veterinary degree and ability to register with the Veterinary Practitioners Board of Victoria
- English language competency in line with registration requirements
- Eligibility for Australian work visa if non-resident

- Prior student placement at equine hospitals within Australia or overseas
- Gain extensive experience of equine surgery, foal and adult medicine, lameness, diagnostic imaging, critical care and anaesthesia through involvement in a large and diverse caseload
- Supervised and supported by specialists and senior veterinarians for on call duties
- Structured and mentored internship program rotating through anaesthesia, medicine, and surgery
- Active involvement in the Group wide continued education
- Provided AVA / EVA memberships and funded CPD allowance
