Overview:
- The State Health Coordination Centre (SHCC) enables systemwide oversight of utilisation of flow of the South Australian health care system. The focus is upon actionable insights that impact the system in a positive way by improving consumer outcomes, experience and community trust.
- The SHCC includes a 24/7 multi-disciplinary clinical team, data and analytics and continuous improvement capabilities. The SHCC systems provide real-time visibility and end-to-end patient flow across the system, working with LHNs and SAAS to support timely flow and address any blocks or barriers.
- SHCC uses the linking of advanced data and analytics with clinical expertise and shared insights to identify issues, patterns and predictions; enabling early response to issues that impact on the system or patient care. By improving coordination, communication, and analytics, the SHCC helps LHNs by supporting progression of care at each stage of their healthcare journey.
- The Advanced Nursing Director will lead, coach, coordinate and support both LHNs and direct reports, with a focus on local and system improvements. They are accountable for standards of patient/client care, and the practice standards of nurses/midwives and/or multi-disciplinary team members. They are responsible for leading the development and ensuring the effectiveness of systems to support, evaluate and consistently improve nursing/midwifery and/or multidisciplinary team practice and healthy work environments.
- The Advanced Nursing Director is accountable for the cost-effective provision of health services within their span of employment and has a high degree of autonomy, independent judgement and decision making.
- The Advanced Nursing Director contributes to the achievement of key performance indicators and deliverables for access and flow across the state and develops a coordinated and integrated approach to manage all systems, processes and redesign imperatives related to patient flow, demand management, and inter-facility flows across all patient continuums within South Australia. This role will work in collaboration with all clinical services, nursing, interdisciplinary and support service colleagues to execute their responsibilities.

About the role
Child and Family Health is a service of the Women's and Children's Health Network, which provides a range of early intervention, parenting and child development programs. These programs support vulnerable families with children 0-3 years, who are experiencing a range of complex issues which may impact on their children's safety, wellbeing and development.
The CaFHS Social Worker works with parents who face challenges through the provision of individual counselling, advocacy and support. The CaFHS Social Worker works as part of a multidisciplinary team to improve the early identification, assessment and care planning for families with complex needs and facing adversity. The Social Worker is based in a CaFHS region but works across sites, including Woodville, Angle Park, Port Adelaide, Ingle Farm and Enfield.
Key duties of the role
- Direct service provision for Collaborative Care eligible clients by providing specialist skills in assessment and engagement of families who are experiencing psychosocial adversity. Appointments may be completed within client homes or clinic-based.
- A positive image of the service will be conveyed through all interactions with clients.
- Takes responsibility for improving the quality of personal practice.
- Contribute to the development and implementation of a service of best practice.
Skills and Experience
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Establish key stakeholder and client relationships across the socio-economic spectrum and from diverse cultures and work effectively within a direct client service environment.
- Proven experience evaluating complex cases and utilising high-level counselling and/or case management practice skills.
- Demonstrated understanding of the major social and health issues facing Aboriginal people, women, fathers and children.
- Dependent on having a current drivers' licence and willingness to drive.

Duties for the role will include:
- Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan.
- Ensuring that requests of the patient’s family re visitors are adhered to.
- Answering all incoming telephone calls and dealing with routine patient enquiries – using judgement in conveying appropriate information.
- Welcoming and directing visitors to the patient concerned
- Monitoring workflow, prioritising work and meeting deadlines.

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. Selection Criteria
To be considered for this position, please ensure you address the below questions as thoroughly as possible.
- Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
- Holds a relevant clinical or education post registration qualifications or such education and clinical experience in Neurosciences.
- Sound understanding of teaching and learning in the health care environment (adult learning principles) and demonstrated ability to apply same in practice.
- Demonstrated effective interpersonal skills, excellent written and verbal communication skills, advanced computer skills and use of software programs to develop and deliver education.
- Demonstrated clinical teaching and facilitation skills.
- Demonstrated knowledge of assessment of practice in a clinical setting.
- Demonstrated commitment to professional development in others and to maintain, improve and broaden knowledge, expertise and competence and capability across a team.
- Current Certificate IV in Training and Assessment (TAE40122) or equivalent; or willingness to obtain within 12 months of commencement in role.

Selection Criteria
- Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife.
- Demonstrated recent clinical experience as a Registered Nurse that applies to an Endoscopy Unit clinical setting
- Demonstrated high level written and verbal communication skills, interpersonal skills and be able to identify key messages, issues and concerns when communicating with others.
- Ability to work as part of an interdisciplinary team and apply professional and ethical boundaries when dealing with complex situations.
- Current computer skills and proven ability to use health information systems.
