Responsibilities
- Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures.
- Provision of professional and clinical leadership for team members
- Ensuring timely allocation of incoming referrals to the appropriate consultant in alignment with DSA KPI's and Activity Work Plan.
- Responsible for office environments including safety, routine maintenance and functioning equipment and ICT.
- Oversees local use and maintenance of fleet vehicles including utilisation, electronic tracking and reporting.
- Responsible to delegated approvals for brokerage and travel, in line with financial delegation and HammondCare Policy.
- Ensure that the wellbeing and safety of team members is considered and proactively managed.
- With support of Senior Team leader maintain local key stake holder relationships
- Provide regular reporting to Regional Manager on all staff performance, service quality, operational performances and issues.
- Recruit team members with appropriate skills for the specific roles being recruited for.
- Effectively manage customer complaints or challenging situations as they arise, with early notification to the Regional Manager, and to the Head of National Programs when needed.
- Work with Regional Manager to address incidents and feedback relating to the quality of the DSA Service

What do we need from you?
- Tertiary qualifications in Nursing/ Allied Health or health- related degree.
- Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management.
- Ability to manage and support a team of skilled professionals across multiple sites and locations.
- Displays strong customer service ethic and attitude Ability to work on multiple tasks and projects simultaneously.

Key Responsibilities
- Achieve sales targets and deliver
- Manage specified sales territories, consistently develop and grow the territory through engagement with existing and new customers;
- This is a customer facing role and it is expected that at least 80% of time will be spent in the field.
- Manage key accounts and target customers.
- Understand customers objectives, buying criteria and decision-making processes to form long standing mutually beneficial partnerships.
- Attend to customer enquiries and needs.
- Develop sales strategies and tactics in conjunction with the National Sales
- Manager, ensuring alignment to the approved marketing plan, as well as implementation of these strategies and tactics.
- Submit monthly sales reports inclusive of sales forecasting and market intelligence including competitor information by the due date.
- Maintain accurate customer account records and customer database including customer targeting within Sales Force.
- Provide product information, demonstrations and training to customers.
- Manage product trials & - when and where required.
- Enhance case management abilities via ongoing development of procedural and product knowledge, i.e. continuous self-learning.
- Following each procedure attended, ensure debrief with Gastroenterologists/Surgeons/NUMs in order to: Establish likes/dislikes regarding product/procedure. Reaffirm their preference for DT products. Obtain additional business. Set (or re-set) their expectations regarding ongoing DT personnel case attendance. Manage Consignment stock. Obtain purchase order and invoice all items used.
Specifically, focus on building meaningful business relationships with customers, given their higher potential for conversion to DT products.
Be available (and suitably organised, i.e. carry appropriate samples etc.) to discuss business growth opportunities with customers whenever the opportunity arises.
Accurately document all valid issues (e.g. product, procedural etc.) in Sales
Force, and pass on information, in a timely fashion, to National Sales Manager.
Monitor Hospital consignment stock levels, and expiry dates and act upon reordering opportunities, comply with all stock keeping requirements per policy.
Ensure all customers are equipped with product collateral as required.
Ensure all sustainable business growth opportunities are followed-up in a timely manner:
Chase-up outstanding as and when required (with a particular focus on end of month)

Nephrology: a speciality with a purpose
Nephrology is a complex and interesting specialty, with a broad variety of clinical conditions and wide range of medical practice - from specialist to generalist, from management of acutely affected patients to a long-term holistic care. Its complexity is not only charming but well respected in internal medicine.
Nephrology provides an intellectually stimulating and challenging working environment. It is defined by the care of patients with diseases of the kidneys and urinary tract. It is comprised of several significant components including general medicine, basic science, clinical research and teaching, as well as the clinical areas of transplantation, dialysis, hypertension, obstetric medicine, and an opportunity for procedural work.
The NSW/ACT Nephrology Advanced Training Program offers three year appointments for trainees within three distinct training networks with the opportunity to preference your preferred networks. Individual training needs to meet training requirements of the College of Physicians, as well as trainee interests, are considered when allocating terms to offer an individualised program for each of the network's trainees.
Where you'll be working
The NSW/ACT Nephrology Advanced network structures ensure trainees have access to more training opportunities across NSW/ACT and provide a superior training experience in line with college requirements.
The Hunter-Western-ACT Network (HWA), comprises of Westmead, Nepean and Blacktown hospitals as part of the Western Sydney Renal Service, the John Hunter Hospital and the Canberra Renal Health Service and offers a comprehensive, diverse, state of the art nephrology clinical training in areas of transplantation, dialysis, renal supportive care, obstetrics medicine, renal genetics, metabolic bone disease, transition care (paediatrics to adult care), rural and regional nephrology. Westmead hospital is the largest kidney pancreas and islet transplant program in Australia. In addition to the clinical activity, the network provides opportunities for research through the Westmead Institute of Medical Research, Centre for Kidney Research/BEAT-CKD Consortium, Hunter Medical Research Institute (HMRI) and the ACT'S center for Health and Medical Research. Opportunities exist for involvement in these research activities both during the course of the transplant rotation and at a postgraduate level. Individual training needs to meet training requirements of the College of Physicians, as well as trainee interests, are considered when allocating terms to offer an individualised program for each of the network's trainees.
The East Coast New England Renal Network (ECNE) offers a broad range of nephrology experience. Trainees will be rotated through 6 monthly terms across several different hospitals within Sydney and across NSW: St George Hospital, Prince of Wales Hospital, Royal North Shore Hospital,Wollongong Hospital, Gosford Hospital and Tamworth Hospital. Each of these hospitals provide excellent teaching and extensive opportunities for managing inpatient and outpatient care in general nephrology, haemodialysis and peritoneal dialysis and collectively they provide exposure to acute and chronic transplant care, renal diseases of pregnancy, renal supportive care, renal stone disease, and interventional nephrology. Individual training needs to meet training requirements of the College of Physicians, as well as trainee interests, are considered when allocating terms to offer an individualised program for each of the network's trainees.
The Central South West Regional Renal Network (CSWR) is composed of facilities including Royal Prince Alfred Hospital, Liverpool Hospital as well as other major Sydney Metropolitan hospitals (Concord, St Vincent's, Campbelltown, and Bankstown) and rural terms at Dubbo Hospital, in Central NSW and Lismore in Northern NSW. A comprehensive training experience is offered in all renal sub-speciality areas within the CSWRN including; obstetric medicine, outpatient dialysis including home-based therapies, renal supportive care, interventional nephrology, renal genetics and acute renal transplantation. The acute renal transplant unit at Royal Prince Alfred Hospital is amongst the largest in Australia. The breadth of facilities within CSWRN means that all sub interest areas of renal medicine are available within the network and the training program can be tailored to a trainee's individual interests and requirements. Rural outreach clinics are also included in some terms where the registrar, under consultant supervision, provides a fly-in fly-out renal service to an area of rural NSW. Individual training needs to meet training requirements of the College ofPhysicians, as well as trainee interests, are considered when allocating terms to offer an individualised program for each of the network's trainees.
What you'll be doing
In Nephrology, you will train under supervision and prepare for independent practice as a consultant. The program builds your skills through work-based learning and assessment tools.
At the end of your training program you will be adept to:
- independently deliver a health service provision and demonstrate a practical understanding of renal diseases/conditions, renal replacement therapies and transplantation
- recognise the impact of chronic kidney disease on individuals, family groups and society
- describe the particular needs of Aboriginal and Torres Strait Islander, Maori and Pacific Islander peoples of Australia and New Zealand
- show an awareness of, and sensitivity to, the special needs of patients from culturally and linguistically diverse backgrounds
- work within, and fully use multidisciplinary team-based approaches to the assessment, management and care of patients
- understand the importance of independent research and ongoing education and training
- be equipped with the skills required to develop independent research projects
- implement future career planning and decision making processes based on a more informed level of knowledge and understanding.
- Qualifications and experience commensurate with the Essential Requirements for the role.
- Completion of at least four postgraduate years and successful completion of the examination requirements of the Royal Australasian College of Physicians, and be eligible to undertake subspecialty training in Nephrology in Australia.
- Demonstrate ability to work effectively as part of a multi-disciplinary team in the planning, delivery and coordination of patient care.

A structured 5-bed, 24-hour residential withdrawal service supporting young people aged 13 to 21 experiencing substance use-related harms. This role involves facilitating withdrawal plans and delivering holistic, strength-based care to young people, supporting their physical withdrawal and overall health needs including sexual and reproductive health, immunisations and follow-up care. The Clinical Nurse will contribute to integrated care plans, participate in group facilitation and support the ADAWS Program Coordinator to ensure a safe, supportive environment for young people from across Queensland.
Clarance Street ADAWS accepts referrals from across Queensland and forms part of the broader Mater Young Adult Health Centre, which offers a continuum of services including a longer-term residential program (Thread), specialist youth outreach, a drop-in Day Program, and parent psycho-education and support through individual, couple, and group interventions.
In this role, you will provide comprehensive clinical services to young people, including assessment, management, health promotion, and coordination of care within a multidisciplinary team.
- Assessing and managing young people with complex and simple health presentations, including intoxication and withdrawal syndromes
- Developing and implementing individual health and withdrawal management plans, including pharmacotherapy support
- Delivering brief interventions, education, and supported referrals to navigate the healthcare system
- Liaising with community health providers to facilitate follow-up care and immunisations
- Participating in multidisciplinary case reviews and contributing to team-based care at Clarence Street Service
- Providing secondary consultation and clinical advice to internal teams and external stakeholders

Be part of a specialist Older Persons Mental Health Service dedicated to supporting older Tasmanians to maintain their mental health, independence and quality of life. You will be working alongside an experienced multidisciplinary team delivering high-quality, person centred care across community, aged care, and health care settings throughout North and Northwest Tasmania.
The RoleThe Clinical Nurse Consultant (CNC) - Rapid Access Service (RAS) pilot is an integral senior clinical leadership role within the Older Persons Mental Health Service, providing timely assessment, consultation, care recommendations, follow-up review and support for older people experiencing behavioural and psychological symptoms of dementia (BPSD) and provide ongoing education on the pilot to Residential Aged Care Homes (RACHs). This position will have oversight of the pilot ensuring the referrals are actioned in a timely manner, accurate data is collected and documented appropriately and that the pilot is operating within the current model of care. Working in partnership with the Consultant Psychiatrist, Team Leader, multidisciplinary team, and key service providers, the CNC plays a key role in ensuring appropriate intervention for older Tasmanians experiencing BPSD in residential aged care homes. Wherever possible, the role supports older people to remain safely in their usual place of residence reducing avoidable hospital presentations and admissions.
The TeamThe Older Persons Mental Health Service (OPMHS) North/Northwest Community Team is a specialised multidisciplinary service supporting older Tasmanians to maintain their mental health, wellbeing, independence, and quality of life within their own homes and communities. As part of Tasmania's Statewide Older Persons Mental Health Service, we provide assessment, treatment, consultation, and support for people experiencing mental illness, behavioural and psychological symptoms of dementia, neurocognitive disorders, Huntington's Disease, and complex mental health needs associated with ageing. Our team includes nursing, psychiatry, psychology, social work, occupational therapy, and allied health professionals who work collaboratively with consumers, families, carers, general practitioners, residential aged care providers, hospitals, and community services across the North and Northwest regions.
The position is located at the Older Persons Community Mental Health Services North and Northwest and will require the successful applicant to travel to different locations across the Northern regions of Tasmania.
You will need- Demonstrated advanced level of clinical expertise specific to BPSD, and knowledge in the delivery and coordination of services similar to RAS, including assessment, triage and client centred care.
- Demonstrated sound working knowledge of leadership principles and an ability to provide clinical leadership and coordination of direct care services, within a multidisciplinary team environment and an ability to work independently with minimal direction.
- Demonstrated high level interpersonal skills, including effective communication, negotiation, and conflict resolution skills, as well as the demonstrated capacity to liaise, negotiate and communicate effectively with a range of internal and external stakeholders.

The Paediatric Cardiology service is part of the Division of Women's & Children's Health and provides inpatient and outpatient Fetal and Paediatric Cardiology services. These services include echocardiography, ECG, Holter monitoring and exercise stress testing for both outpatient clinics and inpatients in the Paediatric ward and Special Care Nursery.
Key responsibilities:Performing and reporting paediatric echocardiography studies, inpatient and outpatient.
Holter Monitor application and removal.
Performing ECG's and Supervised Exercise Stress Testing, Holter Monitor interpretation preferable but not essential.
About you:Post Graduate Diploma in Cardiac Ultrasound, DMU or QUT equivalent.
Current ASAR accreditation.
Experience in Paediatric Cardiac Echocardiography.
