The OpportunityAs a Care Partner within our Carrington at Home service, you will guide older people through their care journey with compassion and expertise - across the Support at Home (SaH) program and the Commonwealth Home Support Program (CHSP) - supporting people to stay safe and independent at home as part of a multidisciplinary team. Responsibilities- Delivering high standard case management & service coordination that is informative and responsive to individual goals and choices.
- Collaborating with participants and representatives to deliver an individual goal plan that supports assessed needs, goals and package resources.
- Working with the Clinical Care Partner to coordinate clinical care delivery within the quality framework.
- Ensuring that service delivery is within participant allocated service budget & managing unspent fund balances for the participant.
- Monitoring service outcomes, ensuring continuous improvement and client satisfaction, and that key KPIs are met
- Supporting all participants with navigating My Aged Care and aged care services as required.
- Participating in an on-call roster (remotely) to ensure timely service provision.
- Qualification in Aged Care, Community Services, or a related field; (or equivalent work experience).
- Experience and knowledge of My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs.
- Demonstrated experience in coordinating care services, managing budgets, and working with diverse clients.
- Have an understanding of the Strengthened Standards, Aged Care Act and related regulatory frameworks.
- Excellent communication and interpersonal skills, with a compassionate and client-focused approach.

T his position supports Local Court Magistrate with the diversion of defendants who have or are suspected of having a mental illness, mental disorder or a cognitive impairment from the Criminal Justice System (Court and Correctional Centres) into the community.
Candidate must hold a relevant post graduate qualification in mental health and a current NSW Drivers Licence.
Clinical Nurse Consultant Grade 2
- Current authority to practice as a Registered Nurse with the Nursing & Midwifery Board of Australia.
- Current full registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.
Occupational Therapist Level 4:
- Must hold qualifications recognised for registration with the Occupational Therapy Board of Australia.
- Must hold general registration with the Occupational Therapy Board of Australia.
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.
- Minimum Bachelor's degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.
Clinical Psychologist:
- Four year degree in psychology, being a three year degree with a fourth year honours in psychology; or a qualification deemed equivalent by the employer.
- Hold general registration with Australian Health Practitioner Regulation Agency (AHPRA).
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.

About the Role
The Emergency Department Nurse Practitioner (EDNP) provides advanced clinical nursing care within a defined scope of practice, working both independently and collaboratively within a multidisciplinary team.
- Provide advanced clinical assessment, diagnosis, treatment and discharge planning within endorsed Nurse Practitioner scope
- Initiate and interpret diagnostic investigations, prescribe medications and implement evidence-based therapeutic interventions
- Lead quality improvement initiatives, clinical protocol development and risk management processes within the Emergency Department

About the Candidate
- Current registration as a Registered Nurse with AHPRA and endorsement as a Nurse Practitioner
- Master's Degree in Emergency Nursing (or equivalent specialty qualification)

- Strong leadership, communication and multidisciplinary collaboration skills
