Access Aged Care is expanding across the Central Coast, and we're looking for experienced nurses who enjoy thinking critically, embracing change and making a genuine impact.
This isn't a traditional aged care nursing role.
You'll work autonomously across residential aged care homes, coordinating care, supporting GPs and specialists, and identifying clinical concerns before they become bigger problems. Every day is different, and as our organisation continues to grow, you'll have the opportunity to grow with us.
If you enjoy using your clinical judgement, learning new technology and working in a role where you are trusted to make decisions, we'd love to hear from you.
Why Our Nurses Choose This Role- Onsite Monday to Friday, 9am to 5pm. No nights, weekends or shift work.
- No manual handling. Focus on clinical decision making rather than bedside tasks.
- High trust and autonomy. You're empowered to make a real difference.
- Variety in your work. Support multiple aged care homes across your local region.
- Ongoing professional development through paid study leave and quality CPD.
- Career opportunities within a rapidly growing national organisation.
- Work on site across residential aged care homes as the Access Aged Care nurse.
- Coordinate care alongside facility nursing teams.
- Facilitate GP and specialist rounds, both in person and via telehealth.
- Complete comprehensive clinical assessments.
- Identify concerns early and coordinate timely intervention.
- Manage referrals, follow up actions and clinical documentation.
- Build trusted relationships with residents, families, GPs and facility staff.
- Confidently use multiple technology platforms to coordinate care and improve clinical outcomes.
Access Telehealth is Australia's leading provider of telehealth supported aged care services.
We combine experienced on site nurses with face to face GPs and telehealth specialists to deliver better outcomes for older Australians.
As we continue to expand, our nurses play an important role in shaping how we deliver care. We're looking for people who are adaptable, curious and excited to work in an organisation that's continually evolving.
What You'll Bring- Diploma with current AHPRA registration and at least two years' post graduate experience.
- Experience in aged care or a similar clinical setting.
- Strong clinical judgement and confidence making independent decisions.
- Excellent communication and relationship building skills.
- Confidence using technology and the ability to quickly learn new systems.
- A flexible, growth mindset with the willingness to work across multiple residential aged care homes as operational needs change.
- Strong organisational skills and the ability to prioritise competing demands.
- Current driver's licence and reliable vehicle.
If you're looking for a role where your clinical thinking is valued, your technology skills are embraced, and you'll have the opportunity to grow alongside a rapidly expanding organisation, we'd love to hear from you.
To learn more about our generous referral program offering up to $10,000 for a successful Geriatrician or GP referral.

The Role- Deliverevidence-based assessment and therapy, supporting NDIS participants to achieve meaningful goals.
- Work from home and see clients face to face locally or via telehealth from time to time
- Collaborate with multidisciplinary allied health teams for better outcomes through coordinated care
For us, flexibility means giving good clinicians room to structure their work sensibly.
That could mean clustering face-to-face appointments across two days and using your third day at home for reports, admin or telehealth.
It could mean working from home in the morning and seeing clients that afternoon - or the reverse. It might mean school drop-off, an earlier finish, or carving out time for your child's sports day event.
And sometimes it's the smaller things: having your coffee at home, putting a load of washing on between reports, or getting dinner started before everyone gets home.
Not every arrangement will be realistic, but if work-life balance matters to you, it's worth a conversation.
Flexibility works both waysWe give our clinicians autonomy, but we place real value on reliability.
Clients often move work, family or arrangements around to make appointments possible, so we expect our clinicians to respect that, communicate well and follow through on commitments.
There are KPIs and expectations, but we don't believe good people need to be micromanaged. If you like having the space to get on with your job while understanding the responsibility that comes with that trust, you'll probably appreciate the way we work.
People who thrive here- Are organised self-starters, comfortable managing their schedules, and priorities
- Value true flexibility
- Value a multi-disciplinary approach to client care
