Our HITH service is dedicated to providing high-quality, patient-centred care through a hospital-based model that improves patient outcomes and enhances the patient experience. You'll play an important role in helping patients receive the right care in the right setting, supporting their recovery while avoiding or reducing the need for an extended hospital stay.
Enjoy the flexibility and autonomy to plan your day. You'll be allocated a patient workload and can use your clinical judgement to prioritise visits, plan an efficient route and organise your time around patients' needs, while remaining connected to the wider HITH team.
Working closely with the Nurse Unit Manager (NUM), HITH medical officers, nursing colleagues, GPs, acute hospital teams and community providers, you'll help coordinate safe, individualised care and support seamless transitions between hospital and home. - Support chronic disease management and provide clear, practical education to patients and their families about their condition, treatment and care plan.
- Liaise with HITH medical officers, GPs and the multidisciplinary team to coordinate individualised care and achieve positive patient outcomes.
- Contribute to quality improvement initiatives and promote safe, evidence-based clinical practice.
About You
- Strong clinical assessment and critical-thinking skills, with the confidence to make sound decisions and deliver safe, evidence-based nursing care.
- Experience in Hospital in the Home, Emergency, Critical Care or a similar acute setting is highly regarded.
- Experience supporting chronic disease management and educating patients to confidently manage their health and care at home.
- Excellent communication skills and a patient-centred approach, with the ability to advocate for patients and work effectively with families and healthcare providers.
- Confidence working independently and prioritising care when you are the sole clinician in a patient's home, while recognising when to seek support.
- Demonstrated computer literacy, including email and electronic patient record systems.
- A commitment to ongoing professional development, reflective practice and continuous improvement.
- Strong organisation and time-management skills, with the ability to plan your workload, prioritise visits and manage competing clinical needs.
- Reporting to the Nurse Unit Manager (NUM), you'll be supported by a dedicated HITH medical and nursing team who share a commitment to delivering high-quality care and achieving the best possible outcomes for patients.
Essential Criteria
- Current registration as a Registered Nurse with AHPRA.
- A valid, unrestricted driver's licence and willingness to travel to patients' homes to deliver hospital-level care in the community.
